My staff has asked a question that has stumped me. :-) We have been getting both units of a double red cell collection. When you antigen type one unit of a double red cell collection can you use the results obtained for the other unit or do you have to repeat the testing on an integral sample from the second unit.

Part of me says that when we do donor testing on the donor specimen collected at the time of donation, we use these results to release both units of a double red cell collection.

Part of me says that even though they share the same unit number, they both should be tested separately using an integral segment obtained from each unit at time of testing.

I am leaning towards having each unit tested, as I would still require a x-match be performed from an integral segment from both units and I retype of both units.

What say you?

Is anyone using a complement-coated control cell to check the rectivity of your polyspecific AHG? We currently use our Coombs control cell (IgG-coated) for a positive control and a screening cell as a negative control, but wonder if we should be checking the C3 reactivity as well. We do not use a monospecific C3 AHG beacuse there is only one commercially available control cell and our volume does not justify a standing order or the tedious process of preparing control cells.

Does it even matter because you have to give antigen negative for previously demonstrating antibodies anyway? The antibody is a big C, patient is 56.

How often does the AABB and or the CAP require we retype patients receiving platelets? Our current policy (inpatients only) states that as long as we have a ABO & RH on the current billing number that we do not have to repeat the testing when providing platelets. When dealing with outpatients our policy is that they be redrawn for an ABO & RH on each admission requiring a platelet transfusion. A problem recently came up because our Cancer Treatment center gives outpatients a series billing number that is good for a week. Recently a patient came in early one week and returned that Friday. Since this is technically an out patient we requested a redraw as per our policy. This caused some disagreement between that blood bank and the Nursing staff, they insisted that the billing number was the same and didn't want to redraw the patient. So now we are reviewing ourpolicy. Any input would greatly be appreciated.
Thanks

1. Check no corruption to traceability data and historical records
2. Check interfaces/ printing etc
3. Check compatibility rules
4. check special interest/warning flags are ok
5. Check stock entry/returns
6. Check input of results
7.Check password access levels for various activities/restrictions.

What other things would I need to considr, not only from the blood bank point of view- but Pathology/IT aspects too?

I am just trying to find out how folks really feel about using checklists in the lab for all key functions, e.g Routine grouping areas, crossmatching bench, 'other miscellaneous' tasks. We have recently introduced new ones to cover things which tended to get overlooked but are actually very important to a well functioning lab, but these are seen to be onerous to staff trying to use them.

What sort of tasks do you add to your checklists- and how do you assign responsibilities to completing these?

I am especially interested in following:daily review of incidents, recall/ quarantine events, inputting referred sample reports, checking antibody panels, checking QC logs/ equipment maintenance logs etc...what some people would call peripheral lab tasks.

thanks!

I'm a fairly recent graduate (less than a year) currently working in the blood bank of a level 1 trauma hospital in the southeastern US. I work 7pm-7am and during our slow periods I've taken to browsing this forum and a few other BB sites in an effort to increase my blood banking knowledge. I've already learned a lot and look forward to learning more.
